LIABILITY - I hereby agree to release and hold harmless, Lauren Dingman and all employees from ANY and all claims, damages, and legal actions arising from, or connected in any way to my tattoo/piercing procedure.
CUSTOMER RESPONSIBILITY - I acknowledge that infection is always possible with any tattoo/piercing and that Lauren Dingman will not be responsible for any complications resulting from not following proper aftercare procedures and/or my lifestyle and behavioral choices following my procedure.
ALLERGIES - If concerned about allergies, I have consulted my doctor and am unaware of any allergies to any pigments/ink, metals, or any products used in the process of either tattooing or piercing. If any such allergies are known, I have stated each in full detail in the MEDICAL CONDITIONS space provided.

SPELLING / LANGUAGE - I acknowledge it is my personal responsibility to ensure any tattoo design containing words, lettering, numbers, or specific info are reviewed before tattoo application. Your artist will show you your stencil and ask you to confirm all spelling and specific info to be included.
